What is a Personal Health Assessment?
A Personal Health Assessment (PHA) is a confidential, self-administered questionnaire that asks questions about your current lifestyle habits, your health history, and current health status. You will log on to Florida Blue to complete this task which takes approximately 15 minutes. You will need to Log In with your Florida Blue Member ID number (starts with an H). You will also have the opportunity to add in your results from the Biometric Screening that was performed at the Health Fair. By completing the Personal Health Assessment, you will be provided information regarding programs and services to help improve your health and well-being.

Health & Wellness, then Better You Strides. - Read and accept the Terms of Service, then choose your
communication preferences.
From the AlwaysOn mobile app:
- Download the AlwaysOn mobile app from the Apple App store or Google Play. Click “New User.”
- Complete the authentication step.
- Create your user name, password and PIN.
What is a Biometric Screening?
The easiest method to assessing your risk for potential health conditions is by measuring:
- Body mass index (BMI)
- Blood pressure
- Total Cholesterol
- High Density Lipoproteins (HDL)
- Total Cholesterol to High Density Lipoprotein ratio
- Blood Glucose
The screening is performed by a skilled professional who will perform a finger stick to obtain a small sample of blood which will then be analyzed using a small machine. The results are available within minutes. The skilled professional will review your results and provide recommendations on how to improve your health and wellness.
Who is eligible? – Those employees and their spouses who have St. Lucie Public School’s health insurance plan through Florida Blue.
Before your Biometric Screening – Individuals are encouraged to fast 8 hours prior to the screening in order to achieve the most accurate results. When fasting, only consume water if necessary.

Or to register by phone, simply call 1-855-623-9355 (M – F: 7:00 a.m. – 8:30 p.m. CST; Sat: 7:30 a.m. – 4:00 p.m. CST)
- Your Unique ID is the first 8 numbers found on your Florida Blue Member Card and your date of birth as MMDDYYYY (ex. Member Card 12345678, DOB 1/7/1965 = 1234567801071965)
- Don’t forget! You shouldn’t eat or drink anything other than water 9-12 hours before your blood draw. Take your medications, including blood pressure medications, with a sip of water. If you have diabetes, please check with your doctor about taking insulin or oral medications if you will be fasting. Be sure to print your confirmation page and take it to your appointment.
- Once your biometric screening is complete, your results will be available online within 3-5 days and a printed copy will be mailed to you
